Care Homes must complete a pre-assessment when a person requires admission to a nursing/care home. This may also be a person returning to their nursing/care home, requiring a reassessment. This assessment ensures the care home can meet that person’s needs.
Delays in hospital discharges across the country have been attributed to delays in assessments.
To address this, the Trusted Assessor model has been implemented nationwide as part of the High Impact Change Model to reduce Delayed Transfers of Care. In the Portsmouth and South East Hampshire region, Hampshire Care Association is overseeing a Trusted Assessor scheme.
Operating from Portsmouth Hospital Trust (QAH), the Trusted Assessor ensures prompt and transparent pre-assessments for all home managers. This role is designed to support home managers by conducting pre-assessments on behalf of care homes across South East Hampshire, Fareham, Gosport, and Portsmouth.
This is a free service to all Care homes within this area.
